is a documentation drafting tool for licensed clinicians. By using the service you agree to these terms.

No patient identifiers

You agree not to submit protected health information (PHI) or any patient identifiers — including names, initials, dates of birth, addresses, phone numbers, medical record numbers, or insurance identifiers — into . The service is designed to work without them.

Clinical responsibility

is a documentation aid. It helps a licensed clinician complete an assessment and narrative in conjunction with that clinician's own findings, the plan of care, physician orders, and other clinical information available to them. It does not assess patients, diagnose, order treatment, provide medical advice, or substitute for clinical judgment.

Every objective finding you sign must come from your own assessment of the patient at that visit. Generated output is a draft: you must read, verify, and correct it, and you sign the finished record in your agency's EMR. does not transmit anything to your EMR on your behalf. You are the clinician of record and remain responsible for the accuracy of the signed record, including that it documents only care actually provided and skilled need that actually exists.

Clinician agreement

When you first sign in you accept a one-time clinician agreement covering these responsibilities. You are not asked again unless the agreement changes, and you can re-read it any time on your profile.

Optional note archive

You may save de-identified generated notes and assessments to your private archive. They are retained for no more than six months unless you permanently delete them sooner; recordings are retained for no more than 30 days. Photos and original shorthand are not archived. You remain responsible for keeping patient identifiers out of all inputs and saved notes.

Membership

Membership is $29 per month after a 7-day free trial. You can cancel anytime; access continues until the end of the paid period. Fees are non-refundable except where required by law.

Acceptable use

Use the service only for lawful clinical documentation. Do not attempt to reverse-engineer the service, share your account, or use it to process PHI.

Liability

The service is provided "as is" without warranties of any kind. To the maximum extent permitted by law, is not liable for documentation decisions, claim denials, or audit outcomes arising from use of generated drafts.
