No PHI is saved or recorded

is built to be used without protected health information. We do not save or record patient names, dates of birth, addresses, phone numbers, medical record or account numbers, insurance identifiers, or exact visit dates. Nothing you bring in is kept as a patient record.

Pages are de-identified on your own device

When you photograph or attach paperwork — physician orders, referrals, face sheets, hospital and discharge packets, medication lists, handwritten visit sheets, prior assessments — the page is read on the device in your hand. Patient identifiers are removed there, before anything is stored or sent. The photo, PDF, or file itself is never uploaded and never stored.

Only the cleaned clinical words are used to help draft your documentation. If you keep the optional tidy-up step on, those cleaned words are straightened out and grouped so reads them better; turn it off and nothing leaves the device at all.

What this does not do

Automatic removal is deliberately aggressive, but no automated process is perfect. Read what pulled off a page before you build from it, and take anything out that should not be there. Do not deliberately dictate or type patient identifiers.

Your responsibility for your own notes

You remain responsible for the security of everything on your own devices and in your own agency systems: your phone or tablet lock, your photo library, downloaded worksheets and PDFs, printed pages, and anything you paste into your EMR. Delete downloaded worksheets when you are done with them, keep your device locked and updated, do not share your sign-in, and follow your agency's policies for handling patient information.

Clinical responsibility

drafts documentation from what you report. It does not diagnose, does not replace your clinical judgment, and does not complete or sign your assessment. You review, correct, and sign everything in your own EMR, and you are responsible for its accuracy.

If this ever changes

If ever begins handling protected health information, this page and our privacy policy will be updated before that happens, and the appropriate agreements would be put in place first.
